Diabetes can cause small amount of protein leak in the urine early on the disease course. This microalbumin creatinine test checks small amount of protein leak in the urine and results are available in 10 minutes. Your provider can adjust your medication based on this instantly available lab result in the office.
Professional Continuous Glucose Monitoring
We offer newer technology for continuous glucose monitoring for diabetes. CGM tracks your glucose levels continuously throughout the day and night. Your glucose reading is sent to† the receiver. We will download the data in 2 weeks with wireless receiver. We can then discuss all the changes in sugar with impact from food and exercise etc. We should be able to help you manage your diabetes treatments.
Diabetic eye exam
Diabetes can affect the eyesight and it is a leading cause of vision problems in the patient with the diabetes. We use smart camera in the office to take the image of eye. It takes only 5 minutes to do the test and results are read by board certified ophthalmologist. This will help prevent and diabetes induced eye damage.
Remote patient monitoring for Diabetes
Normally patients with Diabetes are seen every 3 months in the office. If your sugars are uncontrolled or labile, we offer remote monitoring of your sugar with our smart cellular blood glucose monitors. We provide the device and supplies to the patient. When patient checks sugar at home, we will get the results on your chart instantly. Our provider and dedicated diabetes staff can monitor your sugar and help you control the diabetes better by adjusting the medication in between the office visits.
PharmD Diabetes Medication Management service
Patients with Diabetes needs more help with the medication management and cost of the medication. We can enroll the patient with care management service with our PharmD who then can review all the medications and help with the patient assistance program or pre-authorization of medication. Our office can help you with appropriate referral to the medication assistance program.
Diabetes education service
If our patient needs Diabetes education, we provide them with all the information they need along with appropriate referral to diabetes class and dietitian. We also assign care management staff who will contact the patient monthly to help them educate better about their health condition.
